Residential Manager
Location: Montgomery County, NY
FLSA: Non-Exempt
Reports To: Assistant Director
Physical Requirements: Ability to lift and transfer individuals, walking, climbing, bending and reaching.
Hours of Work: Full time- 40 hours per week. Work hours vary. Frequent evening hours required, minimum 20 hours prior to 8:30 am or beyond 4:30 pm plus one spot-check per week. 24 hour emergency coverage required. Quarterly awake night spot check required. Spot checks conducted at various times, frequency, and shift to ensure unexpected visits. On call responsibilities.
Job Summary: Responsible for managing staff, schedules and finances of the site. Responsible to ensure compliance with all OPWDD & OFPC regulations and state survey results. Ensure that all individuals supported have excellent supports and live the lives they love. Ensure relationships are built that are collaborative and person centered.
Job Qualifications:
1. High School Diploma/GED is required, Bachelor’s Degree is preferred.
2. Minimum 2 years prior experience working directly with individuals and supervising staff functions is required.
3. Medical certification is required.
4. Must be able to read, write and speak the English language.
5. Valid NYS Driver’s license is required.
6. Travel is required; employee must have reliable transportation available.
Major Responsibilities:
1. Monitor all functions of home operation; ensure supports are present for all people in the home.
2. Responsible for the supervision, collaboration, discipline, hiring and termination of direct support staff in the home.
3. Comply with Residential regulations, agency policy and procedures, training and documentation.
4. Maintain budgetary compliance with staff schedules, overtime and household expenses.
5. Ensure accurate documentation and billing requirements are met.
6. Schedule direct support staff and perform direct support duties as needed.
7. Work with family, advocate, or correspondent on behalf of individuals supported in the home.

Senior Habilitation Coordinator
Location: Montgomery County, NY
FLSA: Non-Exempt
Reports To: Assistant Director
Physical Requirements: Lifting: Ability to lift and transfer consumers, bending, pulling, pushing, walking, and pushing wheelchairs.
Hours of Work: Full Time 37.5 hours/wk. Flexible schedule required to provide necessary support.
Job Summary: Oversees Day Support Programs. Responsible for all aspects of scheduling, supervision, training; Habilitation plan oversight and fiscal management.
Job Qualifications:
1. Bachelor’s Degree in Human Services, related field or enrollment in an accredited degree program preferred. High School diploma/GED required.
2. Minimum two years of experience working with individuals with developmental disabilities and/or related human services program required.
3. Must be willing to get medication administration certified.
4. Minimum one year experience supervising staff.
5. Must be able to read and write the English language.
6. Must have valid NYS Driver’s License.
7. Travel is required; employee must have reliable transportation available, as Habilitation Coordinators may be managing more than one program site.
Major Responsibilities:
1. Responsible for the supervision, discipline, hiring and termination of direct support staff.
2. Responsible for program development and oversight built around individual’s personal outcomes.
3. Comply with day habilitation regulations, agency P & P, training, and documentation.
4. Monitor all functions of day habilitation operation; ensure supports are present for all individuals.

6. Responsible for assisting the day program scheduler with scheduling needs for assigned program sites as needed.
7. Perform direct support duties as needed, this varies and is dependent upon site assignments.

10. Responsible for completing and reviewing yearly evaluations and leadership development tools.

Direct Support Professional
Location: Montgomery County, NY
FLSA: Non-Exempt
Reports To: Residential Manager/Family Support Manager/Habilitation Coordinator Physical Requirements: Lifting: Ability to lift and transfer individuals, lift 50lbs, walking, climbing, bending, reaching, kneeling, squatting, twisting. Ability to carry out Strategies for Crisis Intervention and Protection techniques is required.
Hours of Work: Weekend, evening and holiday work is required. Dependent upon assigned residence.
Job Summary:
Ensure all daily care and supports are provided to persons with disabilities in a person-centered manner so that each person lives a life they love. Uphold agency values and work collaboratively with all agency functions to ensure assigned supports are completed to promote health, safety and quality of life for the individuals supported.
Job Qualifications:

Major Responsibilities:
1. Ensure that supports are provided with consideration of each person’s individualized routines and procedures per the individual support plan.
2. Provide a safe, secure, and comfortable home atmosphere.
3. Assist individuals with purchases including choice, expense, responsibility, and planning.
4. Ensure that medical needs are arranged for the individual with as much independence from the individual as possible. Collaborate with nursing staff and execute nursing orders as required.
5. Ensure continuity and security for each individual.
6. Follow agency, state, and federal regulations in order to maintain compliance.
7. Report all incidents, including allegations of abuse and neglect and serious incidents, to the Manager and other required parties immediately upon discovery.
8. Support and train individuals in respect to fire evacuation and incidents of abuse through natural teaching moments.
9. Ensure that individuals participate in the life of the community.
10. Attend staff meetings as scheduled on a monthly basis.
11. Attend training and individual specific in-services as necessary.
12. Ensure that at no time is an individual left without proper coverage or relief, or in an unsafe condition due to staffing needs.

Custodian I (Nights)
Location: Amsterdam, NY
FLSA: Non Exempt
Reports To: Custodian Supervisor
Physical Requirements: Lift up to 50 pounds, standing, bending, walking, and climbing
Hours of Work: Monday thru Friday, 40 hours per week, 4:00 pm – 12:00 am. Additional hours as needed.
Job Summary: Custodian I will be responsible to maintain building appearance and cleanliness at multiple locations, including but not limited to 40 Wall Street, Amsterdam and 47 Liberty Drive, Amsterdam.
Job Qualifications:
1. High school or GED is required.
2. Minimum of 2 years’ experience in custodial services is required.
3. Good working knowledge of cleaning equipment and procedures is preferred.
4. Must be able to read, write and speak the English language.
5. Valid NYS Driver’s license is required.
6. COVID 19 Vaccination required prior to start of work.
Major Responsibilities:
1. Maintain facilities according to specifications for custodial maintenance as assigned.
2. Provide backup for Custodial Supervisor as required.
3. Maintain equipment as agreed.
4. Maintain materials inventory and request materials as necessary.

Lead Clerk (Welcome Center Shift Leader)
Location: Mohawk Valley Welcome Center, Fultonville, NY

Physical Requirements: Standing, sitting, reaching, bending, squatting, kneeling, climbing, lifting, push & pull heavy loads
Hours of Work: 37.5 hours per week. Ability to work a flexible schedule to include weekends.
Job Summary: This position will assist the Site Assistant Manager in all aspects of operations, including sales management, stocking, inventory, customer service and day to day oversight of clerks.
Job Qualifications:
1. 1 year experience preferred in a grocery, deli and/or retail environment.
2. Experience working with individuals with intellectual and developmental disabilities preferred but not required.
3. High School Diploma or equivalent required.
4. Valid driver’s license required.
5. Ability to read, write and speak the English language.
Major Responsibilities:
1. Assumes oversight of store operations in absence of Store Manager.
2. Performs store opening and closing duties.
3. Maintains a high level of customer service.
4. Monitors and occasionally directs work of other store personnel.
5. Assists in inventory control including ordering, deliveries, storage, stocking and display.
6. Complies with store policies.
7. Complies with federal, state, local and store regulations and standards for product freshness, food safety, weights and measures, store safety, employee safety, refrigeration and sanitation.

Relief Medical Office Clerk – Front Desk
Location: New Dimensions in Healthcare - Amsterdam, NY
FLSA: Non-Exempt
Reports To: Front Desk Supervisor
Physical Requirements: Must be able to sit for extended periods of time
Hours of Work: Relief – as needed basis
Job Summary: Performs necessary tasks to schedule patients, complete new patient demographics and check out patients.
Job Qualifications:
 High School Diploma or GED
 Two years’ experience in any combination of the following: medical/dental scheduling, office procedures, reception/front desk, medical records, insurance. Knowledge of medical terminology preferred, with good verbal and written communication skills.
Major Responsibilities:
 Greet and assist in patient sign in/information verification process, be responsive to patient and visitor needs. Determine if appointment exists and indicate patient present for services.
 Receive incoming telephone calls, resolve issues, route calls as indicated or take and distribute messages.
 Perform computerized registration of new patients using information verbally obtained or via intake packet information. Update registration information as appropriate. Collect signatures on all pertinent treatment consent and release of information forms. Distribute privacy, right and responsibility, advanced directive and other information as required at the time of the first patient visit, obtaining necessary signatures verifying receipt of such information.
 Verify payer information on all new patients.
 Periodically, as indicated by Health Center policy, confirm patient eligibility by various payers and update computer system as indicated.
 Schedule, cancel and reschedule appointments as required by provider request and patient need.
 Collect and post payments received through the mail or directly from patients.
 Confirm future appointments.
 Maintain provider schedules in the computer system.
 Prepare records for transfer of information to other health care providers, patients, attorneys and other agencies in keeping with established Health Center policy and all applicable state and federal laws and regulations.
 Analyze records/documents ensuring completeness and flag deficiencies.
 Assist in training of new staff as required.
